Who should complete this survey?
The survey should be completed by the primary caretaker of a child with disabilities. If the primary caretaker cares for more than one child with disabilities, the caretaker should complete one survey for each child cared for. Click here to complete the survey.

Here you will find our letter to providers introducing the project and containing content that you can forward to the caretakers you serve.
Is the survey anonymous?
All responses will be kept anonymous. The information you provide will be used for research purposes only and will not be shared with any third parties.
How will results be shared and what will you do with them?
The results of this survey will be used to inform HOPe's planning for the future of its respite services. We will share the results with our staff, board of directors, state agencies, and community partners. We will also use the results to develop new programs and services to better meet the needs of families.
